parent forms for registration

These forms are necessary in the registration process. During registration you will be asked to confirm with your initials that you have read, are informed of, and when necessary have agreed to the following information shared below.

This form goes over the details of our home school program, which are very helpful to be aware of before a session begins. These topics include: what to bring to class each day, common meeting locations, tardiness/absences, make-up days, rainy days, suggestions for cold/wet weather clothing and for warm weather clothing, and common hazards in the park.


This form goes over details of our Summer Camps, such as where we meet, what to bring to camp each day, cancellation policy, etc.


This form summarizes what our cancelation policies are, and how to avoid any cancellation fees. For homeschool programs only.


The following form will be sent to you after registration, via Adobe. It will need to be signed and submitted back to us. If you would like to preview it’s text before registering, please click the link.

 

This form is an “Assumption of Risk, Release of Liability, and Exculpation Agreement.” This form will be updated for the fall programs to account for the section on Covid19 (the current link is not updated yet).
